Our employee benefit plan consists of Extended Health (prescriptions, vision, medical, paramedical), Dental, Short-Term Disability, Long-Term Disability, Life Insurance, Dependent Life Insurance (spouse and children, if applicable), and Accidental Death and Dismemberment Insurance.

Upon commencement of employment, all employees are required to enrol in the company benefit plan.  You may waive enrolment for Health and Dental coverage only if you already have coverage through your spouse (proof of coverage must be provided).  All employees will be enrolled in Life, AD&D, STD and LTD insurance, regardless of additional coverage.

Our current provider is Equitable Health, and our policy number is 814476.  Your certificate number will be unique to you.

Claims may be submitted online through the provider’s website, or through their mobile app.

There is a 3-month waiting period for all new employees before benefits take effect.

The company pays 100% of your Health and Dental premiums, and the employee pays 100% of their Life, Dependent Life (if applicable), AD&D, STD and LTD premiums.  We’ve set it up like this so that in the event of a disability or life insurance claim, any money you receive will be non-taxable.

Once your benefits are active, instead of taking the full premium payment off one cheque (like many employers do), half of your monthly premium will be deducted from each paycheck to keep things consistent (no premiums will be deducted from anyone during the initial probation period).

Twice per year, there will be a month that has three paydays; in such cases, the premiums will be deducted from your 1st and 2nd cheques – no premiums will be deducted from the first cheque.

A summary of benefits may be found in the top list of links, and a more comprehensive description may be found on the provider’s site in the employee booklet.
