VIOLENCE AND HARASSMENT INVESTIGATION PROCEDURE OUTLINE

This outline is to be followed in conjunction with the Violence & Harassment Report Form, and the Violence & Harassment Investigation Form.

  1. The victim must complete a Violence & Harassment Report (available on the team website, or a paper copy may be obtained from the lunch room).
  2. Once this form is completed, the victim must give the form to the company’s Health & Safety Representative (Diane), or their immediate supervisor, as long as the supervisor is not part of the complaint/investigation.
  3. The Health & Safety Representative will then conduct interviews with the victim, the alleged perpetrator, and any witnesses.
  4. The Health & Safety Representative will then obtain any other pertinent information pertaining to the incident.
  5. The Health & Safety Representative will offer their conclusions regarding the incident.
  6. The Health & Safety Representative will put forth any necessary corrective and preventative actions regarding the incident, delegate said actions to those responsible (if necessary), and set a target date for said actions to be completed.
  7. The Health & Safety Representative will then communicate the final results to the victim, the alleged perpetrator, and management.
